Legal Name Change

We accept applications for legal name changes. We recommend that you visit the  Service Alberta Website for detailed information regarding the requirements and regulations before applying for a legal name change.

The legal name change form must be picked up in person at the registry, as Vital Statistics will only accept applications that are completed on their original forms.
